Private Letters/Forms

If you require a letter from a doctor you will need to book a telephone consultation to request this.  Please note simple letters are chargeable and are payable in advance.

If you have a form that needs completing by a doctor please drop these at reception by prior arrangement.  These will be sent to the secretaries who will contact you with the appropriate fee.  Please note the fee is payable before completion.

Referrals

Referral Process

If you think you need to be referred to a hospital on an NHS or Privately basis you will need to make an appointment to speak to the doctor. The doctor will make a referral based on their clinical decision, please do not make a private appointment before you have spoken to the doctor.

E-Referral

If the doctor refers you on the NHS this may be an Electronic referral and the paperwork will be given to you by the doctor in the surgery, via a text message if you have a smart phone or emailed to you.

You can then arrange your own appointment either by phone or electronically.  If no appointments are available to you please click or ask to be deferred to the provider this will put you on the hospital list to be contacted when appointments are available.

Learning Disabilities Health Checks

We offer an annual health check for patients with learning disability needs. This is running slightly differently this year due to COVID-19.  To limit the amount of time spent in the surgery it will begin with a telephone consultation with one of the Advanced nurse practitioners followed by a short appointment in the surgery. Patients will be contacted to arrange this.

Diabetic and Pre-Diabetic clinics

The surgery continues with a commitment to help and support you to improve your control and reduce risks.

The surgery will see you annually for a blood test to check your kidneys, cholesterol and sugars and these results are followed up with you by the Diabetic Practice nurse and any adjustments to your medication made plus lifestyle support and advice with referrals and signposting to other areas that may be of benefit, ie Desmond Education course.

This annual check for anyone with diabetes will include a foot check to ensure any problems with skin or circulation, which can cause problems in diabetes patients, are found and treated as soon as possible. Advice in general foot check will also be given.

As well as this annual full check, for those with diabetes, a check on sugar control every 6 months also helps to support and improve ongoing control and reduce possible complications in the future.

COPD Monitoring

​The surgery offers annual reviews for people who take inhalers for the respiratory conditions asthma and COPD  [Chronic obstructive Pulmonary disease].

The clinics with the Practice nurse will review symptoms, inhaler use and technique and the experience nurse will assess any change of treatment to improve symptoms/condition.

Although these reviews are generally annually, if you are having any problems or concerns with your inhalers or symptoms between these reviews then you can contact the surgery and book with the clinician for review.

Contraception Advice

The surgery offers appointments with the Practice nurse for the continuation of prescribed oral and injectable forms of contraception, together with advice or sign posting to other sexual health services.

If you are looking to start a contraception, then an appointment with one of the GP’s can be made to advise and initiate this. Please contact the surgery.

The surgery also offers an Implant insertion and removal service by a GP if you decide that this would be a good option for your contraception.

Please note the surgery doesn’t not provide a coil fitting service, but we can signpost you to the sexual health services that would be able to.

Chronic Heart Disease/Hypertension Monitoring

The surgery provides ongoing monitoring of blood pressure of people on medication for their blood pressure or heart. The surgery also offers an annual review of your cholesterol and kidney function by way of a blood test to help advise with the lowering of any other risk factors and help the Clinical Pharmacist with their annual review of your medication to ensure safety and control.

The surgery will contact you when your review is due but if you have any concerns with regards your Blood Pressure or medication please don’t hesitate to contact the surgery to arrange a review with a clinician or nurse.

Childhood Immunisations

It is important to keep your child’s vaccinations and protection up to date….First baby health check and baby vaccines are at 8 weeks of age, followed by a further 2 lots at 12 and 16 weeks of age. The vaccination programme continues with boosters at 1yr and 3 years 4mths [pre school].

You will be contacted by the surgery when your child is due any of these vaccinations but if you are unsure if your child has missed any then please contact the surgery and this can be checked and appts made to give missing vaccines made.

Cervical Smears

Cervical screening or smears as it’s otherwise known, are very important and is recommended for all ladies/people with a cervix, from the age of 24 and 6mths , until 65yrs age.

You will receive a letter from the local health authority when your smear is due but please contact the surgery if you are in doubt when you last had one to ensure you keep up to date.

We understand that the procedure of cervical screening can cause some anxiety and embarrassment and result in some people delaying or declining to have the test done, so if you would like to know more about the procedure or ask some questions about the test then please contact the surgery and the nurse can help answer any concerns you may have.

Although cervical screening is a routine screening if you feel that you are experiencing any abnormal symptoms, i.e. irregular bleeding, abdominal pain, abnormal discharge, and are either under 24yrs and 6mths or between routine smear testing, then please contact the surgery and make an appointment to discuss these symptoms with one of the Clinicians.
