The surgery provides ongoing monitoring of blood pressure of people on medication for their blood pressure or heart. The surgery also offers an annual review of your cholesterol and kidney function by way of a blood test to help advise with the lowering of any other risk factors and help the Clinical Pharmacist with their annual review of your medication to ensure safety and control.
The surgery will contact you when your review is due but if you have any concerns with regards your Blood Pressure or medication please don’t hesitate to contact the surgery to arrange a review with a clinician or nurse.
JUNE 2020….Medication reviews are done over the telephone but the surgery is open for blood pressure checks if you are unable to monitor your blood pressure at home.
